Nurse’s Office Info.:

For Students Interested in Playing a Fall 2019 Sport for Copper Beech M.S.

- Check with the School Nurse to see if you have a current physical exam on file in the Nurse’s Office.  Physical exams must be dated on or after September 1, 2018, to be valid.

-Sports registration is completed online and found on the Lakeland Central School District website.  Registration opens on Friday, August 9, 2019, and closes on Friday, August 16, 2019, at 3:00 p.m.  If you do not register your child online by August 16th, this may jeopardize your child’s sports participation.  Online forms must be completed by a parent/guardian ONLY.

-For those selected students who are applying for participation in a high school sport (APP), you will need to complete the Sports Card on the main page of the Lakeland District website, starting on Friday, July 26, 2019, and closing on Thursday, August 8th, 2019, at 9:00 a.m.  You MUST complete the Sports Card within this time frame in order to play for a high school sport.  Please make sure you apply for the online Sports Card for the high school your child will be attending in the future:  Lakeland High School or Walter Panas High School.

Attention 6th Grade Parents:  As per New York State Education Law, all students entering 7th Grade for the 2019/2020 school year must provide the School Nurse with a copy of the student’s most recent physical exam.  A physical exam is considered valid if it is dated on or after September 4, 2018.  If your child has had an exam that falls within this time frame, then please forward the exam results to our Nurse’s Office, since this will satisfy the NYS requirement.  In addition to a physical exam, all 7th grade students MUST receive the Meningococcal Vaccination as per NYS Department of Health guidelines.  This vaccine must be received before the first day of school in September of 2019, and admittance to school will be denied should your child not have this vaccination.  If your child has not received one as of yet, please contact your MD to make an appointment for your child to receive a vaccination.  Once received, please forward the physical exam and vaccination to the Nurse’s Office.

Attention 8th Grade Parents:  As per New York State Education Law, all students entering 9th grade in 2019/2020, must provide the School Nurse with a copy of the student’s most recent physical exam.  A physical exam is considered valid if it is dated on or after September 4, 2018.  If your child has had an exam that falls within this time frame, then please forward the exam to the Nurse’s Office at Lakeland Copper Beech.  If you do not forward it to the Copper Beech Nurse’s Office this year, you will then need to send it to the School Nurse at the high school your child will be attending next year.

Medication Pick Up from Nurse’s Office:  Student medications must be picked up by a parent/guardian or designated adult no later than Tuesday, June 25th from the Nurse’s Office.  Any medication that is not picked up from school will be disposed of.
