Health Office Information

As per NYS Education Law, all students currently in 7th Grade must submit an updated physical exam. Please submit the student’s most recent physical exam to the Nurse's Office if you have not done so already. All 7th grade students are required by NYS law to be screened for vision and hearing tests in school.  This process will be taking place during school hours.  Parents will be notified should follow up be required.

Reminder all medical notes are to be provided to the Nurse's Office only.  This includes any and all medical exemptions from physical activities in gym class.

If a student has a severe food allergy or requires any medication at school, please provide the completed forms and medications to the Nurse ASAP.  Medication forms can be retrieved from the LCBMS website under Health Office, Health Office Forms, and then the link for:  “Authorization for Medication Administration”.

New York State mandates that all 6th, 7th and 8th grade students attending public school be screened for scoliosis. Scoliosis is the abnormal curve of the spine, and if left untreated, can cause future health problems. If your child has had a physical exam with their health care provider, and if "Scoliosis" is marked on the exam, this will be acceptable. Please keep in mind we have received many physical exams WITHOUT scoliosis marked either positive or negative.  In these cases your child will be evaluated.

Winter Sports Alert:

The online sports card registration has closed.  The Winter sports season does not begin until November 20, 2017.  As sports cards are processed, parents will receive a confirmation email, once all personnel review and complete these cards.  Do not discard this confirmation email, as it is proof that your student’s sports card was approved.  This is a paperless system – sports cards do not need to be printed.  Coaches will have the names of athletes with approved sports cards.
