RE: Administrative Regulations For Home Instruction

The following district procedures/guidelines are to be utilized in the provision of home instruction services

A. Procedures for Home Instruction Due to Medical Absence (upon medical certification of absence for one or more weeks due to illness or injury)

• Elementary students are to receive 1 hour per day.
• Middle & high school students are to receive 2 hours per day.

1. Home instruction should begin on the first day of absence or as soon as practical.
2. It is the responsibility of the middle/high school guidance counselor or the elementary principal to forward the required home Instruction form to the parent (form attached).
3. The parent is responsible for having the combined medical/parent request form completed and returned to the school (guidance counselor/principal).
4. Guidance counselor/principal will forward the signed medical/parent request form to the school nurse who will fax same to the Lakeland Schools’ Physician, Dr. Schaff, @ (914) 737-5979.
5. Dr. Schaff will indicate approval status for the expected duration of the illness. If additional time is needed, the family will need to submit additional medical/parent request form(s), or the family doctor should speak directly with Dr. Schaff (914) 737-6565.
6. Dr. Schaff will forward his decision to the office of the Director of Grants, Programs, & Testing.
7. The office of the Director of Grants, Programs & Testing will forward a copy of the approved (or disapproved) request to both the school se and guidance counselor/principal.
8. When the approved medical/parent request has been received by the guidance counselor/principal, home instruction may begin; approval from the Director of Grants, Programs & Testing must be received by guidance/principal before instruction/tutoring begins. Vouchers received for home instruction prior to approval will not be processed.
9. Home instruction is always calculated In relation to student eligibility, and the hours may be utilized In the future if there has been a delay (i.e., receipt of the medical/parent request form, hiring staff, etc.).
10. The school principal, whose signature indicates that the Instructional Plan and Attendance Report for the student is on file with either the guidance counselor or the principal’s designee, must sign the Home Instruction Voucher.
11. Forward the signed Home Instruction Voucher to the office of the Director of Grants, Programs & Testing. Payment will not be issued without the signature of the Director.
12. Instructional Plans and Attendance Reports are to be filed in student’s permanent school record folder.
13. Every effort should be made to utilize district staff from the home Instruction list. However, in cases where there is difficulty in acquiring services of a tutor, private tutoring agencies such as Creative Tutoring, Inc., (914) 997-1569, may be called. Call only when teachers from the district are unavailable.

B. Procedures for Home instruction Due to Suspension
 
• Elementary students are to receive 1 hour per day.
• Middle & high school students are to receive 2 hours per day.
 
1. Home instruction should begin on the first day of suspension or as soon as practical (but not later than the third day).
2. It is the responsibility of the middle/high school guidance counselor or the elementary principal to forward the Home Instruction Information form to the office of the Director of Grants, Programs, & Testing. (At the high school level, it is the responsibility of the Assistant Principal to complete the top portion of the Home Instruction Information Form-Due to Suspension and forward it to the guidance counselor to arrange for home instruction services.)
3. Home instruction is always calculated in relation to student eligibility, and the hours may be utilized in the future if there has been a delay (i.e., receipt of the request form, hiring staff).
4. The school principal, whose signature indicates that the Instructional Plan and Attendance Report for the student is on file with either the guidance counselor or the principal’s designee, must sign the Home Instruction Voucher.
5. Forward the signed Home Instruction Voucher to the office of the Director of Grants, Programs & Testing. Payment will not be issued without the signature of the Director.

C. Procedures for Home Instruction Due to Special Education Placement

• Elementary students are to receive a minimum of 1 hour per day and any related services identified in the IEP.
• Middle & high school students are to receive a minimum of 2 hours per day and any
related services identified in the IEP.
 
1. Home instruction should begin on the first day as mandated on the IEP.
2. It is the responsibility of the middle/high school guidance counselor or the elementary principal or the special education supervisor to forward the required home Instruction form to the office of the Director of Grants, Programs & Testing and the office of PPS.
3. Home Instruction is always calculated in relation to student eligibility, and the hours may be utilized in the future if there has been a delay (i.e., receipt of the request form, hiring staff).
4. The Director of PPS, whose signature indicates that the IEP and/or Instructional Plan and Attendance Report for the student is on file with either the guidance counselor or Committee for Special Education, must sign the Home Instruction Voucher,
5. Forward the signed Tutor Voucher to the office of the Director of Grants, Programs & Testing. Payment will not be issued without the signature of the Director.
6. Instructional Plans and Attendance Reports are to be filed in student’s permanent school record.
7. Every effort should be made to utilize district staff from the home Instruction list. However, in cases where there is difficulty in acquiring services of a tutor, private tutoring agencies such as Creative Tutoring, Inc., (914) 997-1569, may be called. Call only when teachers from the district are unavailable.
 

D. Rules for Home Instruction

1. Never leave the student alone. Students who are being instructed on school premises must be supervised at all times.
2. The student is to be dismissed only at the site of the instruction services and only to Lakeland’s transportation personnel or the authorized family member. Under NO circumstances is the student to be given permission to go elsewhere for transportation unless authorized in writing by a family member and under no circumstances should the tutor transport the student.
3. If the instructor must leave before the student’s transportation provider has arrived, the Instructor must notify on-site Lakeland personnel who w arrange for supervision of the student.
4. When instructing students at home, an adult must be present in the home when instruction takes place.
5. Home instruction can be delivered one-to-one or in small groups.
6. When possible, home Instruction should end with the last day of Instruction of the school year.
